Pulp therapy refers to a group of treatments that address the soft tissue inside a tooth, known as the pulp, when decay or trauma has affected it. The two most common forms in primary teeth are the pulpotomy and the pulpectomy. A pulpotomy removes only the affected portion of the pulp in the crown of the tooth while leaving healthy tissue in the roots intact, and it is typically used when decay has reached the pulp but has not caused deeper infection.

A pulpectomy removes the entire pulp from both the crown and the root canals, an approach similar to the root canal therapy we provide for adult patients, and is reserved for cases where the infection has spread further. Research published through the National Library of Medicine reviewed outcomes for pulp therapy in primary teeth using a minimum twelve-month follow-up period and found that pulpotomies using mineral trioxide aggregate reached a success rate of 89.6 percent, with formocresol close behind at 85.0 percent. These findings reflect why pulp therapy has become a trusted, well-documented option for preserving primary teeth rather than removing them outright.

Why We Try to Save the Tooth First

Baby teeth do more than fill a smile. They hold space for permanent teeth, guide proper jaw development, and support a child’s ability to chew and speak clearly. When a primary tooth is lost too early, the surrounding teeth can shift into that space, which sometimes crowds out the permanent tooth that was supposed to grow in there.

Pulp therapy allows us to address the infection or damage while keeping the tooth in place for as long as it is still needed. This is especially important for molars, which often are not replaced by permanent teeth until a child is ten to twelve years old. We always weigh pulp therapy against extraction based on how much healthy tooth structure remains and how much time is left before the tooth would naturally come out on its own. Good preventive dental care throughout childhood is one of the best ways to catch decay early, before it ever reaches the pulp.

What to Expect During the Visit

We know that dental visits can feel intimidating for children, so we focus on making the appointment as calm and predictable as possible. Before beginning, we numb the area thoroughly with local anesthetic, and for children who need extra help staying relaxed, we offer nitrous oxide as our sole sedation option for pediatric patients. We do not use oral sedation for children, which keeps the experience simpler and easier to monitor from start to finish.

During the procedure, we remove the decayed or affected pulp tissue, clean the space, and place a medicated dressing designed to protect the remaining healthy tissue and support the tooth’s ongoing function. Music often plays quietly in the treatment room, a small touch that helps put younger patients at ease. Once the pulp therapy is complete, the tooth typically needs a crown to restore its strength, and for pediatric patients, we use stainless steel crowns exclusively rather than zirconia options.

Recovery and Aftercare

Most children return to normal activities, including eating and talking, within a day of treatment. Some mild soreness around the gum line is common as the local anesthetic wears off, and over-the-counter pain relief formulated for children is usually enough to manage any discomfort. We ask parents to keep an eye on the treated tooth over the following weeks and to let us know if they notice swelling, ongoing pain, or any change in the tooth’s color, as these can be signs that additional care is needed.

Regular follow-up visits let us confirm the tooth is healing as expected and give us a chance to catch any issues early.
